    What is a Referring Community Agency?

    A referring community agency is any organization or entity that identifies a person in need and connects them to a Coordinated Care Agency program for support and assistance.

    What Does a Lead Community Health Advocate Do?

    Lead Community Health Advocates are the main point of contact for clients in the Pathways program. They guide clients through the program, connecting them with resources like housing or healthcare, and help them navigate any related processes.

    What does a Community Health Advocate Intern Do?

    Community Health Advocate Interns act as resource detectives. They research and find helpful programs for clients, like housing or food assistance. They keep information up-to-date and share it with the Lead Advocate, who then helps clients access those resources.

    What Does the Social Work Intern Do?
